NY Connects offers free, unbiased information about long-term services and supports in New York State! The service is offered to people of all ages or with any type of disability, including medical and non-medical services needed to improve/maintain health and independence. NY Connects staff meets with clients wherever it is most comfortable for them.

NY Connects offers:

  • Referrals
  • Assistance
  • Personalized counseling

Who does NY Connects serve?

  • Children with disabilities
  • Adults with disabilities
  • Older adults
  • Caregivers and families
  • Providers and helping professionals
  • Those seeking to transition back to the community from a facility

These services may be provided in a nursing home, the individual’s home, or other community-based settings.

Our NY Connects staff can help link to long-term services and supports, such as:

  • home care
  • transportation
  • meals

Our goal is to help our clients live as independently as they wish while meeting the medical, social and functional needs that arise from aging or disability.

Examples of situations where NY Connects can help:

  • If a client requires services after hospital discharge
  • If a client needs assistance with cooking meals, eating, dressing and/or bathing
  • If a client needs assistance finding assistive devices and/or help using them
  • If a recently widowed spouse feels unsafe at home alone
  • If a family needs help with a child who has special needs
