If you would like more control over the in-home services you receive, CDPAP is designed to offer you this opportunity.
To be eligible for CDPAP, you must be:
- A resident of a county in New York State
- Eligible for Medicaid Long Term Care
- Approved to receive personal assistance by your Medicaid Long Term Care Provider
- Willing and able to make informed decisions — or have a designated representative — to oversee your home care services including the hiring, scheduling, supervising and possible termination of personal care aides
Call NYIA (New York Independent Assessor) at 855-222-8350 to schedule two initial assessments: one with a NYIA nurse and one with a NYIA practitioner.
After you are approved, your care plan and the number of hours of in-home services will be determined through an assessment completed by a nurse from your Managed Long-Term Care (MLTC) provider.
CDPAP services in New York State are administered through Public Partnerships LLC (PPL). FLIC is an approved PPL Facilitator.
